Hospice Care

Hospice is a service provided to a patient and their loved ones. This service is provided in the patient’s place of residence. Quality of life becomes the focus of care.

Services Provided:

Hospice services are provided by Registered Nurses, Therapists, Social Workers, Hospice Aides, Chaplains and a Medical Director. Together, they develop an individualized care plan for each patient. We can assist with the following services:

  • Pain management and symptom control
  • Medical care by our team
  • Bereavement support
  • Community resource support
  • Dietary counseling
  • Physical, occupational, speech, and respiratory therapy
  • Volunteer support to assist loved ones
  • Chaplain services
  • Social work and counseling services
